As of April 15, 2024, a significant update to the Minnesota guardianship statutes has been enacted, affecting all guardians in the state of Minnesota. This new regulation mandates that guardians must file annual Personal Well-Being Reports and Affidavits of Service using the MyMNGuardian (MMG) online application.

A Personal Well-Being Report is a document that guardians must file annually to report on the condition and status of the individual under their guardianship. This report covers various aspects of the ward’s life, including: 

  • Health and medical care: updates on the person subject to the guardianship’s health, medical treatments, and any changes in their physical or mental condition. 
  • Living conditions: information on where the person is living, the safety and appropriateness of the environment, and any changes in living arrangements. 
  • Social activities: Details about the person’s social interactions, community activities, and personal relationships. 
  • Personal care: an overview of the daily care and personal assistance the person receives. 

These reports are crucial as they provide a comprehensive overview of the ward’s quality of life and help ensure that their needs are being met appropriately under the guardianship.

Personal Well-Being Reports for people subject to guardianships are essential for several reasons:
